Your Tailor Made Senior Service

Your Tailor Made Senior Service is a Veteran owned and operated provider of in-home care, offering tailor made services to meet the unique needs of your loved ones.We help seniors and other adults who need assistance to live, and thrive, in the home environment they love.

In home care Provided is not limited to:
· Professionally trained caregivers
· Structured activities geared to seniors’ individual interests to engage them physically,mentally, and emotionally.
· Assistance with chores like cooking, cleaning, bathing, dressing, and shopping.
· 24/7 monitoring and notification services
· Medication reminders
· Meal preparation and dietary assistance
· Post-procedure monitoring
· Transportation to appointments and events
·  In-home safety assessments

Your Tailor Made Senior Service and  Veteran Community Partnership The Community Residential Care (CRC) program is for Veterans who do not need hospital or nursing home care but cannot live alone because of medical or psychiatric conditions. Aid and AttendanceAid and Attendance is intended for those who need custodial long-term care or the aid and attendance of another.

Veteran Community Partnerships (VCPs) are organized partnerships through which VA facilities can connect with Your Tailor Made Senior Service to :·    

  • Develop and foster strong relationships.

  • Enhance and improve access to care, services, and benefits.

  • Promote seamless transitions.      

  • Measure results through shared reporting

Your Tailor Made Senior Service works with the VA to Connect Veterans, their families, and caregivers.

When you refer to us, we assist your vets and do all the work for you.

  1. Adding home care to your care plan can reduce avoidable readmissions and emergency room visits.

  2. Discharge planners, social workers, case managers simply refer veterans to Your Tailor Made Senior Service Your Tailor Made Senior Service  includes ongoing client services to help clients remain in VA compliance with medical expenses and to transition to a higher level of care if needed.

  3. We help veterans decide if they should apply for VA Aid and Attendance, complete VA paperwork and get care started right away before VA entitlement begins.

  4. Skilled and Unskilled Home Health Care is for Veterans needing short-term care as they are moving from a hospital or nursing home back to their home.Your tailor Made Senior Service provides continuing care to people with ongoing needs.

  5. Our program is for Veterans who need skilled and unskilled  services such as: skilled nursing, respite, homemaker, home health, case management, physical therapy, occupational therapy, speech therapy, wound care, or IV antibiotics.

  6. Skilled Home Health Care can be used in combination with other Home and Community Based Services.

  7. Alzheimer’s & Dementia Home CareOur specially trained caregivers engage veterans  in intellectual, physical, and emotional interactions that complement medical treatment and improve the quality of life for vets and their families.

We can help care for veterans at home, giving them the support, they need .We believe that everyone should experience connection, purpose and uplifting moments, no matter what their diagnosis is.

Military Screening Questions

4. Are you, or your loved one, a veteran or the surviving spouse of a deceased wartime veteran who needs help to live safely at home? (Surviving spouse must have been married to the veteran at least 1 year and at time of veteran's death. Married couples are only eligible if the veteran is disabled and meets the guidelines.)

5. Did the veteran receive an honorable or general discharge?

6. Did the veteran serve at least one day during wartime? (Combat duty or overseas service is not  
    (required.)

Word War II: December 7, 1941 -- December 31, 1946

Korean Conflict: June 27, 1950 -- January 31, 1955

Vietnam Era: Nov 1, 1955-May 7, 1975. Veterans that served

after August 5, 1964 may have served outside the Republic of Vietnam.

Persian Gulf War: August 2, 1990 -- until date to be

determined

7. Did the veteran serve at least 90 days on active duty? (Persian Gulf War veterans must have 2
    years of active duty or the full period for which they were called for active duty.)

​Medical Screening Questions

8. Does the veteran or surviving spouse have a medical condition causing the need for assistance 
     with some activities of daily living?

(Examples are: help with bathing, dressing, grooming, using the bathroom, meal preparation, and moving about safely.)

9. Is the veteran or surviving spouse 65 years old or older? (You may also check “yes” if younger than 65, and has a letter from Social Security proving a disability.)

10. Does the veteran or surviving spouse want to use his/her VA Pension with Aid & Attendance primarily for in-home care or adult daycare?

(In-home care is non medical care such as help with bathing, dressing, light housekeeping, meal preparation, medication reminders and more.)

Monetary Screening Questions

11. Does the Veteran or Surviving Spouse already receive any monetary VA benefits that is not a military retirement pension? (Examples are Service Connected Disability Pension or Dependency and Indemnity Compensation, commonly called “DIC”?

12. Does he/she or this couple have a total net worth (income & assets) at or less than $150,538?

(A primary residence and vehicle are not counted as part of net worth. Ongoing, un-reimbursed medical expenses and long-term care expenses can be deducted from net worth. If known, please indicate monthly income and estimated total assets in the comments below.)
